Information for Our Patients

   

Our staff is committed to helping you understand and fulfill your insurance requirements so that you can get the medical attention you need. We work with a number of health insurance plans, as well as Medicare. If you do not have health insurance, our staff will assist you in making arrangements based on your financial situation. Payment plans are an option for all patients, regardless of insurance coverage. Please call our office if you have any questions.

We will make every attempt to accommodate your financial needs. No-interest extended payment plans are a common way to make your share of medical costs affordable. We always strive to meet your financial needs to enable you to obtain the best medical care possible!

If you have insurance through a managed care (PPO) plan
Please make sure to bring your insurance card with you to your appointment. Each time you visit our office, you will need to pay the co-payment amount your insurance plan requires. Our office will then file a claim to your insurance company. You will receive a statement from us if any balance is due after we have received reimbursement.

Insurance Plan Notice
Our insurance plan participation list changes each January because reimbursement contracts are renegotiated annually and benefits are altered according to employer contracts. You may find that your deductibles and out-of-pocket expenses may change. It is our policy to treat all patients with the same high quality care regardless of their insurance coverage or lack thereof.

If we're not in your network
If your insurance plan is not listed above, please call or email us, as our participation status with insurance carriers often changes.

For patients with PPO insurance coverage in an insurance plan not listed above, please be advised that your insurance plans may offer the option to seek medical care from out-of-network or nonparticipating providers. Typically, benefits are subject to the plan deductible and then paid at the out-of-network benefit level for coverage. Once the patient’s annual out-of-pocket maximum is met, benefits are considered for payment at 100% of the plan’s usual, customary and reasonable allowance (UCR). Our office will accept the insurance plan’s UCR allowance for each service billed. Patients will not be billed for the amounts deemed over and above the insurance plan’s UCR allowance. Patients will only receive a billing statement for amounts not paid by insurance due to deductible, coinsurance or other insurance non-coverage issues. This is where no-interest payment plans or other options are applied.

If you do not have insurance
Patients without medical insurance are requested to pay at the time of service. A 15% discount will be honored for all services that are paid in full at the time of service. If you cannot pay for medical services in full, you may consult with a member of our business office staff to arrange a payment plan.
